OET reflects real workplace tasks and relevant language skills. Doctors applying for positions in the United States can validate their English language proficiency with OET, the world’s only international English language test specifically for healthcare professionals. The test has been formally recognized by the Educational Commission for Foreign Medical Graduates & the Foundation for Advancement of International Medical Education and Research (ECFMG®|FAIMER®).

Demonstration of English language proficiency always has been a requirement for ECFMG Certification. Residency program directors rely on ECFMG Certification as an indicator that international medical graduates (IMGs) are ready to enter accredited U.S. graduate medical education (GME); this readiness includes English language proficiency.

Since 2004, the English language component of Step 2 Clinical Skills (CS) has been used to meet this requirement. All applicants who are pursuing ECFMG Certification through one of these pathways, regardless of native language, the language of instruction at medical school, or citizenship, must take and achieve a satisfactory score on the OET.

• ECFMG Certification evaluates whether international medical graduates (IMGs) are ready to enter residency or fellowship programs in the US that are accredited by the Accreditation Council for Graduate Medical Education and to obtain a license to practice from a Medical Licensing Authority.

• ECFMG has identified pathways for meeting the clinical skills requirements for ECFMG Certification for IMGs seeking to enter accredited U.S. residencies or fellowships in 2021 as a result of the suspension of Step 2 Clinical Skills (CS) component of the United States Medical Licensing Examination® (USMLE®) due to Covid-19.
